I am disabled, how do I apply?
Application forms are available from libraries and Council Access Points
You must be resident in Scotland and aged between 5 and 59 years. You will be eligible if you meet one of the following criteria;

If you are receiving the higher rate of the mobility component of the Disability Living Allowance (DLA) - please bring with you a current original award letter from Department of Work and Pensions or confirmation of award
on form DBD 384 dated within the last 12 months

If you are receiving the higher or middle rate of the care component of DLA - please bring with you a current original award letter from DWP or confirmation of award on form DBD 384 dated within the last 12 months
-
If you are receiving Attendance Allowance - please bring with you a current original award letter from DWP or confirmation of award on form DBD 384 dated within the last 12 months

If you are receiving war pensioners mobility supplement please bring with you a current original award notice or letter of confirmation from the Veterans Agency dated within the
last 12 months

If you are living in a care or residential home or hospital and are eligible for the higher rate or middle rate of the care component of DLA or Attendance Allowance - please bring with you a current original letter from the DWP or confirmation of award on form DBD 384 dated within the last 12 months

If you have a current
Blue Badge or the latest
V11 vehicle licensing reminder from the DVLA showing the disabled taxation class or form
DLA404 from DWP confirming
higher rate mobility component of the DLA

If your application for a driving licence has been refused or revoked due to being unable to meet the fitness standards needed
to drive. This does not apply to persistent misuse of drugs or alcohol. Please bring with you a DVLA letter reference number
D206, D235, D220 and NS57.

If your ability to travel is impaired by a mental disorder and the disorder has lasted for more than one year and you need
to travel to health or social care appointments or to other activities which are part of your treatment. Please bring with
you a Mental Health Issues Certificate (NCT002) completed and signed by an appropriate professional.

If you are terminally ill or have progressively degenerative disorder which impedes your mobility and stops you carrying out day to day activities
and are receiving the higher rate of the mobility component of DLA or the higher or middle rate of the care component of DLA (DBD 384) . If you are not receiving these allowances please bring with you a copy of a DS1500 report or a Hospice admittance
letter confirming your condition

If you have lost a limb - one or both lower limbs; one lower and one upper limb or both upper limbs and are receiving higher rate of the mobility component of DLA or the higher or middle rate of the care component of DLA (DBD 384. If you are not receiving these allowances please bring with you written confirmation from your prosthetics clinic/centre,
physiotherapist or rehabilitation centre.

How do I apply if I am visually impaired?
If you are registered as blind or partially sighted please bring with you written evidence from the RNIB or a registered local
Blind Society (Grampian Society for the Blind) confirming that you are registered as blind or partially sighted.

How do I apply if I am profoundly or severely deaf?
You qualify for Scotland-Wide free Bus travel if you are profoundly or severely deaf. You will need a letter from an Audiologist,
Hospital Consultant or Aberdeen and North East Deaf Society confirming the above. Your letter must state that you have bilateral
hearing loss greater than 70dBHLs averaged over 1, 2 and 4KHz.

I am unable to travel by bus without assistance. Can the person accompanying me get free bus travel?
You may be able to get free bus travel for your companion if you meet one of the following :

You are receiving or have been awarded the higher or the middle rate of the care component of DLA. You will need to bring
your current original award letter DWP or confirmation of award on form DBD 384.

You live in a care home, residential home or hospital and you are eligible for the higher or the middle rate of the care component
of DLA or Attendance Allowance. You will need to bring your original award letter DWP or confirmation of award on form DBD
384.

You are registered as blind with the Grampian Society for the Blind see Scottish Blind Persons Scheme
When you return your completed application form please bring with you a new passport style photograph; proof of address as
well as the proof of entitlement listed above.
